We collect information from you through treatment, payment, healthcare operations, applications, and enrollment processes. This includes:

  • Personal Information: Name, phone number, email address, and other details provided during interactions with our office.
  • Protected Health Information (PHI): Any information (oral, written, or recorded) created or received by healthcare entities (providers, hospitals, pharmacies, health plans). This includes data relating to your past, present, or future treatment, payment for healthcare, or enrollment in a health plan that contains your name, address, SSN, insurance number, or other identifiers.

  • Treatment Chain: Sexton Vision Group and our Essilor/Luxxotica affiliated partners (LensCrafters, Pearle Vision) are both “Providers.” We share patient information as part of the “treatment chain” for the benefit and convenience of our patients.
  • Minimum Necessary: We limit disclosures to the minimum necessary to conduct business and be of service to our patients. We make reasonable efforts to minimize “incidental disclosures.”

If you provide your mobile phone number, you may receive SMS (text) messages related to your care.

  • Service Messages: Includes appointment reminders, scheduling updates, and service-related notifications.
  • Marketing Messages: Sent only if you have explicitly opted in to receive them.
  • Terms: Message frequency may vary. Message and data rates may apply.
  • Opt-Out: You may opt out at any time by replying STOP. For assistance, reply HELP.
  • Privacy: We do not share or sell your mobile phone number or personal information to third parties for marketing purposes.

  • No Sale of Data: At no point will your PHI or personal information be sold or shared with third parties for marketing or commercial use.

  • Confidential Communications: You have the right to receive confidential communications of your PHI by alternative means or at alternative locations.
  • Requests: We may require written requests for alternative communications. We must accommodate reasonable requests if you clearly state that disclosure of the information would endanger you.
  • No Explanation Required: We will not require you to provide an explanation for the basis of your request as a condition of providing confidential communications.
